Very interesting that the former owner of a medical urgent care company is making concessions for the health insurers. We need more protection for doctors against the insurance company monopolies, I think as doctors we see right through this. The heart of the matter is what doctors are worth, and it is very clear that more and more the insurance companies are deciding that for everyone.

I will quit my practice of 29 years if I am forced to deal with another bureaucratic regulation. I do not have time to deal with all of the administrative 'bean counting' hassles. I do not have time to take care of the sick patients, who seem to need more and more attention for their problems.

Dear Tam, you are obviously confused about the current role of a ARNP. Perhaps you do not realize that Florida and Alabama are currently the only states in the United States that do not allow ARNPs to prescribe narcotic medications. Also, several independent studies have been conducted to evaluate the "prescribing habits" of those APRNs who are allowed to write for prescription narcotics, and it has been proven APRNs are more judicious than most general practice physicians when prescribing. Yes, you are correct, currently - there is a requirement for ARPNs to have a collaborative agreement with a physician. Again, Florida is one of only a few states that requires this agreement. ARNPs are not claiming to be a medical doctor, however, many are doctors they hold either a Phd or DNP. By the year 2015 it will be a requirement ARNPs have a doctoral degree. So, this should make you feel a little better knowing that while your waiting to see a MD (that can't see you because he's so busy) - you will still be seen by a "real doctor" an APRN. Have a great day; )

shall i be jailed or fined for asking if my gunshot-wound patient has a gun at home? what about the psychiatrist who is gauging his patient's suicidal risk? it is atrocious that a doctor has to worry about going to jail if he asks the wrong question to his patient. He can, alternatively, be sued for not protecting his patient or others. the patient does not have to answer if he feels it infringes on his rights.

You fail to mention that physicians are now required to be 'data entry' technicians for the EMR system. We do not have enough time to take care of patients, much less enter specific data points. For me, I elect to not perform this time consuming task, and actually take care of patients. Do any physicians actually listen to their patients or examine them anymore? If they do, they do not have time to 'buff an electronic chart', which the government is surely going to inspect.

What about Electronic Prescribing of Controlled Substances (EPCS)? I hear that there is limited actual usage due to DEA's stringent requirements (such as two-factor authentication).
